Legal Insurance

SEIU Local 503 members are eligible for group legal insurance through the ARAG Insurance Company. The Ultimate Legal Insurance plan offers a toll-free telephone legal advice hotline, and will cover your attorney fees for wills, divorce, adoption, bankruptcy, financial education and counseling services, and many other covered services.

Members may enroll for coverage within 90 days of joining SEIU Local 503, OPEU or during an open enrollment period.

Legal Insurance Enrollment Form
Use this form to enroll in legal insurance during open enrollment or within 90 days of new SEIU Local 503 membership. You can also use this form to change your legal insurance enrollment.

Life and Legal Domestic Partner Affidavit
Submit this form along with your life or legal insurance enrollment form to insure your domestic partner.

Summary Annual Report
This document must be available to all participants in the member life and disability and legal plan at the end of the financial year. This report contains financial and related information about the plan.

Life and Disability Insurance

All members of Local 503 automatically receive $2,500 in free life insurance. You are automatically enrolled when you become a member of SEIU Local 503. The coverage continues as long as you are a dues-paying SEIU Local 503 member.

Members also have the opportunity to purchase additional life and disability insurance through the Regence Life and Health Insurance Company. Members must work at least 80 hours per month to qualify.

New members who elect coverage within 90 days of becoming a member may enroll for either $17,000 or $34,000 of supplemental life insurance without having to answer health questions. Members are then eligible to increase their supplemental life to the next benefit level during Open Enrollment yearly, up to a maximum of $69,000. Please refer to your policy for details.

Supplemental policies include an accidental death and dismemberment policy of three times your benefit amount.

Members may enroll in Short Term Disability without purchasing supplemental life insurance. Evidence of insurability is still required.  Coverage becomes effective once approved and payment is received.

Dependent life insurance for your spouse/domestic partner and children in the amounts of $2,500 or $5,000 is also available, but additional information may be required.

Aflac

Aflac offers guaranteed-renewable insurance policies, including accident, hospital confinement indemnity, dental, vision, and cancer/specified-disease policies. The benefits are predetermined and paid in addition to any other insurance you have.

Aflac is pleased to offer these insurance policies to SEIU Local 503 members. These policies may be the help you need if you are sick or injured. SEIU members can apply for these policies at payroll rates. You can even have the premium deducted from your checking or savings account, or charged to your credit card. We will meet with you in a confidential one-on-one setting to discuss the policies and determine which will best fit YOUR needs. We may ask a number of questions during our time together to help you make that determination. We want you to have the coverage you need to help protect you and your family. Several of the policies offer a choice of different levels of coverage. We will look at your present and future needs together to select the right coverage for you and your family.

Oregon Prescription Drug Program (OPDP)

Oregonians with no health insurance or those who have insurance but could use help paying for prescriptions can take advantage of the discount prices available through OPDP resulting in lower prescription drug prices. There is no age or income limit for this free program.
