What documentation do I need to apply for Health Kids?
·       Proof of income required for current and previous month (pay stubs, letter from employer, etc.)
·       No citizen or identity documentation is required for children born in Oregon and age 15 or younger
·       Children born in Oregon and age 16-18 must show proof of identity (State issued drivers license or ID card, student picture ID, etc.)
·       An original birth certificate is required for children born in the US outside of Oregon
·       Additional documentation is required for children born outside of the US
 
Is there a waiting list for Healthy Kids?
No, other state health plans have waiting lists but not Healthy Kids.
 
My children were previously denied for the Oregon Health Plan. Should I reapply for Healthy Kid
Absolutely.
 
Which medical providers are under the Healthy Kids plan?
All children will have access to a provider network based on the county in which they live.
 
What is our union’s interest in Healthy Kids?

SEIU Local 503 has a special relationship with the Healthy Kids program. Hundreds of our members lobbied to win passage of Healthy Kids and other healthcare access and funding initiatives. Our Union applied for and was awarded a grant from Healthy Kids to do outreach to hard-to-reach children in Oregon and to assist in the application process.
